Intraoperative Arterial Hypertension in Noncardiac Surgery : Controversies and Current Recommendations

چکیده

15 Abstract Arterial hypertension on the day of surgery is a frequently encountered phenomenon nowadays. In many patients, correction of the elevated blood pressure (BP) helps to avoid serious complications and improves perioperative safety. However, there is no general agreement on whether all patients presenting with high BP will benefit from an antihypertensive therapy prior to the start of anesthesia and throughout surgery. In addition, there is no consensus on what should be considered a baseline “normal” value for BP in patients undergoing surgery. This report focuses on controversies in management of intraoperative hypertension and emphasizes the importance of preventing labile BP changes rather than attempting to keep BP within a predetermined target range.

Intraoperative Mean Arterial Pressure Variability and 30-day Mortality in Patients Having Noncardiac Surgery.

BACKGROUND Little is known about the relationship between intraoperative blood pressure variability and mortality after noncardiac surgery. Therefore, the authors tested the hypothesis that blood pressure variability, independent from absolute blood pressure, is associated with increased 30-day mortality. METHODS Baseline and intraoperative variables plus 30-day mortality were obtained for 10...

A case of severe pulmonary hypertension associated with common arterial trunk and VSD in a 9 years old child

Background: Common arterial trunk (CAT) is a rare congenital heart disease, and often leads to the early development of pulmonary hypertension and disability. Among the critical structural heart defects, the frequency of CAT is 3%, which reflects the severe hemodynamic disturbances. The natural course of the disease is characterized by a high mortality rate up to 88% during the first year of li...

Optimal perioperative management of arterial blood pressure

Perioperative blood pressure management is a key factor of patient care for anesthetists, as perioperative hemodynamic instability is associated with cardiovascular complications. Hypertension is an independent predictive factor of cardiac adverse events in noncardiac surgery. Intraoperative hypotension is one of the most encountered factors associated with death related to anesthesia. In the p...

Preoperative preparation of patients with arterial or pulmonary hypertension in noncardiac surgery.

Arterial hypertension is not an independent risk factor in cardiovascular complications in noncardiac surgery. Nevertheless, preoperative evaluation is necessary and includes estimation of arterial hypertension grade and possible damage of target organs. In patients with first and second grade of arterial hypertension postponement of elective intervention is not necessary, only optimization of ...

Arterial pulmonary hypertension in noncardiac intensive care unit

Pulmonary artery pressure elevation complicates the course of many complex disorders treated in a noncardiac intensive care unit. Acute pulmonary hypertension, however, remains underdiagnosed and its treatment frequently begins only after serious complications have developed. Significant pathophysiologic differences between acute and chronic pulmonary hypertension make current classification an...
